Forms to register your child for soccer

There are several methods you can use to register your child with Nampa KID Soccer:

1. Online registration
   We have recently added the ability to register and pay online. If the link is active in the Registration Menu on the front page, you may select that option and pay by PayPal or any major credit card.
2. Mail in form
   When you register online, you'll be given the choice to pay by mail. Go ahead and fill out the form online, then print the invoice and mail it with your check to the following address:

3. Scholarship form
   Print this form (en Español) and mail it to the above address to have your scholarship application reviewed.
